Composition

Active ingredients: 

betamethasone dipropionate in terms of 100% substance - 0.064 g, 

clotrimazole in terms of 100% substance - 1 g, 

gentamicin sulfate in terms of gentamicin - 0.1 g; 

 

Excipients: 

propyl parahydroxybenzoate (nipazole), 

liquid paraffin (liquid paraffin), 

isopropyl myristate,

 petrolatum.

pharmachologic effect

Combined preparation for external use with anti-inflammatory, anti-allergic, antifungal and antibacterial action.

Gentamicin is a broad-spectrum antibiotic of bactericidal action from the group of aminoglycosides, highly active against gram-negative bacteria Proteus spp. (indole-positive and indole-negative strains), Escherichia coli, Klebsiella spp., Salmonella spp., Shigella spp., Campylobacter spp., Staphylococcus spp. (methicillin-resistant). Active against Serratia spp., Pseudomonas spp., Acinetobacter spp., Citrobacter spp. Resistant to gentamicin: Neisseria meningitidis, Treponema pallidum, anaerobic microorganisms (Streptococcus spp. (Except Strepnococcus pneumoniae), Providencia rettgeri).

Betamethasone has anti-inflammatory, anti-allergic, anti-exudative effects.

Clotrimazole  is an antifungal agent from the group of imidazole derivatives. It acts by disrupting the synthesis of ergosterol, which is an integral part of the cell membrane of fungi. Possesses a wide spectrum of action. It is active against pathogenic dermatophytes (Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, Microsporum canis), yeast and mold fungi (Candida albicans, Turolopsis glabrata, Rhodotorula spp., Pityrosporum orbiculare).

Pharmacokinetics

With external use of the drug in therapeutic doses, the transdermal absorption of active substances into the blood is very insignificant. When applied to the skin, the absorption rate of betamethasone depends on the state of the epidermal barrier (inflammation and skin diseases increase absorption).

The use of occlusive dressings increases the transdermal absorption of betamethasone and gentamicin, which may lead to an increased risk of systemic side effects.

Indications

- simple and allergic dermatitis (especially complicated by secondary infection);

- diffuse neurodermatitis (including atopic dermatitis);

- limited neurodermatitis (including simple chronic lichen);

- eczema;

- dermatomycosis (dermatophytosis, candidiasis, versicolor versicolor), especially when localized in the groin and large folds of the skin.

Application during pregnancy and lactation

During pregnancy (especially in the first trimester), topical application of the ointment in pregnant women is allowed only in cases where the intended benefit to the mother outweighs the potential risk to the fetus. In such cases, the use of the drug should be short-lived and limited to small areas of the skin. 

It is not known whether the components of the drug are excreted in breast milk. Therefore, when prescribing Akriderm GK ointment during lactation, it is recommended to stop breastfeeding.

Contraindications

- tuberculosis of the skin;

- cutaneous manifestations of syphilis;

- chicken pox;

- herpes simplex;

- post-vaccination skin reactions;

- open wounds;

- children under 2 years old;

- hypersensitivity to drug components.

With caution: the drug should be used during pregnancy, especially in the first trimester, in children aged 2 to 18 years.

Side effects

Itching, burning, irritation, dry skin, folliculitis, hypertrichosis, steroid acne, hypo-pigmentation, allergic reactions. When using occlusive dressings - maceration, infection, skin atrophy, striae, prickly heat. With prolonged treatment or application to a large surface, the development of systemic side effects: increased body weight, osteoporosis, increased blood pressure, edema, ulceration of the mucous membrane of the gastrointestinal tract, exacerbation of hidden foci of infection, hyperglycemia, agitation, insomnia, dysmenorrhea.

Interaction

The interaction of Akriderma GK with other drugs has not been established.

How to take, course of administration and dosage

Outwardly. 

Ointment Akrider GK is applied to the affected skin, lightly rubbing, a small amount 2 times a day. The duration of treatment is determined individually and depends on the nosological form and severity of the disease. With dermatomycosis of the feet, the average duration of treatment is 2-4 weeks. If clinical improvement does not occur in the near future, it is necessary to clarify the diagnosis or change the treatment regimen.

Overdose

Symptoms:  phenomena of hypercortisolism. 

Treatment:  gradual withdrawal of the drug. Symptomatic therapy. If necessary, correction of electrolyte disturbances.

Special instructions

Avoid contact with eyes. 


When a stable bacterial or fungal microflora appears, the drug should be canceled and appropriate therapy should be prescribed. 


Due to the fact that in children the area of ​​the skin in relation to body weight is larger than in adults, as well as the epidermis is insufficiently developed, with external use of the drug, absorption of a proportionally larger amount of active substances is possible and, therefore, there is a greater risk of developing systemic side effects ... The drug should be used in children as soon as possible and in compliance with all precautions.
