Composition

1 g of ointment contains:

Active substances:

betamethasone dipropionate 0.64 mg;

salicylic acid 30 mg;

Excipients:

liquid paraffin;

petrolatum.

pharmachologic effect

Diprosalik is an anti-inflammatory, antipruritic, antiallergic, keratolytic, antimicrobial agent. Diprosalik is a combined preparation based on two active substances - betamethasone dipropionate and salicylic acid.

Betamethasone dipropionate is a glucocorticosteroid that has anti-inflammatory, anti-allergic and antipruritic effects. 

Salicylic acid, when applied topically, has keratolytic as well as antimicrobial and antifungal properties.

Indications

To reduce the inflammatory manifestations of dry and hyperkeratic dermatoses sensitive to GCS therapy, including:

  • psoriasis;
  • chronic atopic dermatitis;
  • neurodermatitis (chronic lichen simplex);
  • lichen planus;
  • eczema (coin-shaped, hand eczema, eczematous dermatitis);
  • dyshidrosis;
  • seborrheic dermatitis of the scalp;
  • ichthyosis and ichthyosis lesions.

Application during pregnancy and lactation

Due to the fact that the safety of using local GCS during pregnancy has not been established, the appointment of this class of drugs is justified only if the potential benefit to the mother outweighs the potential risk to the fetus.

Drugs in this class should not be used during pregnancy in high doses or for a long time.

Since it has not yet been clarified whether the level of systemic absorption of GCS is sufficient for the appearance of their detectable concentrations in breast milk, either breastfeeding or the use of the drug should be discontinued during treatment, taking into account how necessary it is for the mother.

Contraindications

Hypersensitivity to any component of the drug.

Side effects

Burning, itching, irritation and dryness of the skin, folliculitis, hypertrichosis, acne-like rashes, hypopigmentation, perioral dermatitis, allergic contact dermatitis; when using occlusive dressings - maceration and atrophy of the skin, secondary infection, striae, prickly heat.

Salicylic acid preparations can cause dermatitis.

With local application of GCS for a long period and / or at high doses, especially in children, side effects characteristic of systemic GCS may develop (including suppression of the function of the hypothalamic-pituitary-adrenal system, the development of Cushing's syndrome, growth retardation, insufficient weight gain and increased intracranial pressure).

How to take, course of administration and dosage

Apply in a thin layer 2 times a day - in the morning and at night, completely covering the affected skin.

In some patients, a supportive effect can be achieved with less frequent applications.

The duration of the course of treatment is determined by its effectiveness, as well as by the patient's tolerance, the presence and severity of side effects.

Overdose

Symptoms: excessive or prolonged use of corticosteroids for topical application can cause suppression of the function of the hypothalamic-pituitary-adrenal system, leading to secondary adrenal insufficiency and hypercortisolism, including Cushing's disease.

Prolonged use of topical salicylic acid preparations in high doses can cause symptoms of salicylate poisoning (early symptoms are nausea and vomiting).

Treatment: symptomatic therapy. Acute symptoms of hypercortisolism are usually reversible. If necessary, correct the electrolyte imbalance.

In case of poisoning with salicylic acid, it is necessary to take measures to quickly remove salicylic acid from the body, for example, ingestion of sodium bicarbonate (to alkalinize urine and force diuresis).

Special instructions

If dryness, skin irritation or hypersensitivity occurs, the drug should be discontinued. In case of infection, it is necessary to prescribe appropriate therapy. With local application of GCS, especially in children, side effects characteristic of systemic GCS, incl. suppression of the function of the adrenal cortex.

Systemic absorption of GCS and salicylic acid when applied topically will be higher when using occlusive dressings, as well as if extensive body surfaces are treated. Appropriate precautions must be taken if prolonged use of drugs is expected (especially in children).

With long-term treatment, drug withdrawal should be carried out gradually.

The ointment and lotion are not intended for ophthalmic use. Avoid contact with eyes and mucous membranes.

Do not apply lotion under occlusive dressings.

Do not apply the drug to an open wound or damaged skin.

Application in pediatrics

The drug can be used with caution in children over 2 years of age.

Children are more at risk of suppression of the function of the hypothalamic-pituitary-adrenal system due to the use of local GCS than adults, due to their higher ratio of body surface area to weight and, accordingly, increased absorption of the drug.

There were reports of suppression of the function of the hypothalamic-pituitary-adrenal system, Cushing's syndrome, growth retardation, lag in weight gain, increased intracranial pressure in children who received topical GCS.

Suppression of the function of the hypothalamic-pituitary-adrenal system in children is determined by low plasma cortisol levels and a lack of response to ACTH stimulation. Increased intracranial pressure is manifested by protrusion of the fontanel, headaches, bilateral edema of the optic nerve head.
