Product Overview
Composition
Active substance:
polidocanol.
pharmachologic effect
Ethoxysclerol has a venosclerosing, local anesthetic effect. Damages the vascular endothelium, causing protein denaturation, stimulates the rapid formation of a thrombus and its organization within 7 days.
Ethoxysclerol reduces the excitability of nerve endings and the conductivity of the pain impulse, providing painless sclerotherapy.
Pharmacokinetics
The volume of distribution is 24.5 liters, binding to plasma proteins is 64%, T1 / 2 is 4 hours. Total Cl - 11.7 l / h, renal - 2.43 l / h, biliary - 3.14 l / h. With repeated administrations, it does not cumulate.
Indications
Symptomatic treatment of functional asthenia.
Application during pregnancy and lactation
Contraindicated in the first trimester of pregnancy and after 36 weeks;
Breastfeeding after the introduction of Ethoxysclerol is recommended to be stopped for 2-3 days.
Contraindications
Hypersensitivity to the drug Ethoxysclerol, bronchial asthma, diabetes mellitus
Side effects
When sclerosing varicose veins of the extremities: hyperpigmentation of the skin at the injection site of Ethoxysclerol.
When sclerosing hemorrhoids: transient pain.
When sclerosing varicose veins of the esophagus and stomach: necrosis, ulceration and stenosis of the esophagus, exudation into the pleural cavity, dysphagia, fever, anaphylactic shock.
Interaction
Enhances the arrhythmogenic activity of local anesthetics and drugs for general anesthesia.
How to take, course of administration and dosage
Ethoxysclerol is administered strictly intravenously with the leg horizontal or raised by 30-45 °.
The total dose of polidocanol for sclerosing varicose veins and hemorrhoids should not exceed 2 mg / kg of body weight per day.
For a patient weighing 70 kg, up to 140 mg of polidocanol can be administered (28 ml of 0.5% ethoxysclerol solution, 14 ml of 1% ethoxysclerol solution, 4.6 ml of 3% ethoxysclerol solution).
Overdose
Symptoms: with paravasal administration - necrosis.
Treatment: administration of 0.5-2 ml of 1% procaine solution or 5-20 ml of 0.9% NaCl solution in combination with hyaluronidase.
Special instructions
CHF in the compensation phase and arterial hypertension of a stable course are not contraindications for sclerotherapy.
Sclerosing drugs should not be administered intravenously, because this can lead to severe necrosis and forced amputation. In case of accidental intravenous administration, inject 5-10 ml of 1-2% lidocaine or mepivacaine solution, 500 IU of heparin into the needle left in the artery; wrap the limb with cotton wool and lower it down; hospitalize the patient in the vascular surgery department.
The introduction of sclerosing drugs into the veins of the facial part of the skull is carried out only according to strict indications.
