pharmachologic effect
The drug Festal compensates for the insufficiency of exocrine pancreatic function due to pancreatin, and biliary function of the liver due to the bile component. Festal has a proteolytic, amylolytic and lipolytic effect.
The enzymes that are part of pancreatin: amylase, lipase and protease facilitate the digestion of carbohydrates, fats and proteins, which contributes to their more complete absorption in the small intestine. And in this regard, Festal is practically no different from other enzyme preparations.
The presence of bile acids in the Festival gives the drug additional opportunities for the correction of biliary insufficiency, which is often associated with chronic pancreatitis. Bile extract acts choleretic, facilitates the absorption of fats and fat-soluble vitamins A, E and K, promotes the release of lipase by the pancreas.
The hemicellulase enzyme promotes the breakdown of plant fiber, which also improves digestive processes, reduces the formation of gases in the intestine.
Indications
Lack of exocrine pancreatic function in chronic pancreatitis in combination with biliary insufficiency due to various pathological conditions, clinically manifested by food digestion disorders, flatulence, a tendency to constipation, as part of combination therapy for:
- diffuse liver diseases - alcoholic and toxic liver damage, cirrhosis;
- large losses of bile acids (in patients after cholecystectomy);
- violation of the circulation of bile acids, observed with dyskinesia. biliary tract, dysbiosis, malabsorption;
- violation of neurohumoral regulation of bile formation and bile secretion in chronic diseases of the gastrointestinal tract - chronic gastritis, chronic duodenitis, chronic cholecystitis
To improve the digestion of food in patients with normal gastrointestinal function in case of errors in nutrition, as well as violations of chewing function, forced prolonged immobilization, a sedentary lifestyle.
Preparation for x-ray and ultrasound examination of the abdominal organs.
Pregnancy and lactation
Perhaps the use of the drug during pregnancy and lactation (breastfeeding) according to indications.
Contraindications
- Hypersensitivity to the components of the drug Festal,
- acute pancreatitis
- exacerbation of chronic pancreatitis,
- liver failure,
- hepatic coma or precoma,
- hepatitis,
- hyperbilirubinemia,
- obstructive jaundice
- cholelithiasis,
- empyema of the gallbladder,
- intestinal obstruction,
- tendency to diarrhea,
- children's age up to 3 years (for this dosage form).
Precautions: cystic fibrosis, pregnancy.
Side effects
Allergic reactions (flushing of the skin, sneezing, lacrimation), nausea, diarrhea, abdominal pain (including intestinal colic).
With prolonged use in high doses, the development of hyperuricemia, hyperuricuria is possible.
In children (when taken in large doses), the development of perianal irritation and irritation of the oral mucosa is possible.
Interaction
With simultaneous use with Festal, the absorption of PASK (p-aminosalicylic acid), sulfonamides, antibiotics is enhanced; a decrease in absorption of iron preparations is possible.
The simultaneous use of antacids containing calcium carbonate and / or magnesium hydroxide can lead to a decrease in the effectiveness of Festal. Cimetidine enhances the effect of the drug.
How to take, course of administration and dosage
Inside.
The drug Festal is taken orally, without chewing, during or immediately after a meal, with a small amount of liquid. Assign 1-2 tablets 3 times a day.
Higher doses are prescribed only by a doctor.
The dosage and dosage regimen in children is determined by the doctor.
The duration of treatment can vary from a few days (if the digestive process is disturbed due to errors in the diet) to several months or years (if constant replacement therapy is necessary).
Before x-ray or ultrasound examination - 2 tablets 2-3 times a day for 2-3 days before the study.
Overdose
Symptoms of an overdose are hyperuricemia and hyperuricuria.
If symptoms of an overdose appear, discontinue the drug and conduct symptomatic therapy.
Special instructions
With caution, the drug is prescribed for patients with a history of chronic pancreatitis.
It should be borne in mind that with high activity of the lipase contained in pancreatin, the likelihood of developing severe constipation in children increases.
In clinical practice, there have been cases of fibrous colonopathy in children with cystic fibrosis who received high-dose pancreatin preparations daily.
Digestive disorders can occur in patients with hypersensitivity to pancreatin, or in patients with a history of meconium ileus or intestinal resection.
When using pancreatin in high doses in some cases, the occurrence of perianal irritation is possible.
