pharmachologic effect

Guttalax Express is a local laxative from the group of diphenylmethane derivatives with a slight antiresorptive effect. After hydrolysis in the large intestine, bisacodyl stimulates its motility and promotes the accumulation of water and electrolytes in the lumen of the large intestine. This leads to stimulation of bowel movements, a decrease in evacuation time and softening of the stool.

The action is due to direct stimulation of nerve endings in the mucous membrane of the large intestine.

The developmental time of the laxative effect of the drug is 20 minutes, in some cases 45 minutes.

Bisacodyl, being a laxative acting at the level of the colon, stimulates the natural process of evacuation in the lower parts of the gastrointestinal tract.
 
Therefore, bisacodyl does not affect the digestion or absorption of high-calorie foods or essential nutrients in the small intestine.

Indications

  • Constipation due to hypotension of the colon (including chronic constipation, regulation of stool in the postpartum period, in bedridden patients);
  • preparation for instrumental and radiological studies; 
  • preoperative preparation, postoperative period.

Recommendations for use

For constipation

Adults and children over 10 years old - 1 suppository per day.

Do not exceed the maximum recommended daily dose (10 mg).

The suppository is inserted into the rectum with the pointed end forward.

The action occurs in about 20 minutes (from 10 minutes to 30 minutes).

Do not use the drug daily without consulting a doctor and establishing the cause of constipation for more than 10 days.

In preparation for research, pre- and postoperative treatment

In preparation for research, pre- and postoperative treatment, medical conditions that require easier bowel movements, the drug should be used under medical supervision. It is recommended in combination with tablets to achieve complete emptying. In addition to 2-4 tablets taken the day before, one suppository is recommended in the morning.

The decision on the required number of tablets of the drug to be taken on the eve of the study / operation is made by the attending physician, based on the clinical condition of the patient and the amount of upcoming manipulation.

Contraindications

  • Hypersensitivity to the active ingredient or excipients;
  • intestinal obstruction;
  • obstructive bowel disease;
  • acute diseases of the abdominal organs, including appendicitis;
  • acute inflammatory bowel disease;
  • severe abdominal pain associated with nausea and vomiting, which may be a sign of a more serious condition;
  • severe dehydration;
  • children's age up to 10 years.

Carefully:

Hepatic and / or renal failure, advanced age.

Side effects

The most commonly reported adverse reactions during the use of the drug are spastic abdominal pain and diarrhea. 
From the gastrointestinal tract: spastic pain or discomfort in the abdomen, diarrhea (dehydration, muscle weakness, cramps, decreased blood pressure), nausea, vomiting, a small amount of blood in the feces, discomfort in the rectum, and colitis.
 
The use of suppositories may be accompanied by pain and local irritation, especially with anal fissure or ulcerative proctitis.
 
From the nervous system: dizziness, fainting. These side effects that occur after the use of the drug are associated with a vasovagal reaction (i.e. due to intestinal spasm, tension during bowel movements).
 
On the part of the immune system: hypersensitivity, angioedema, anaphylactic reactions.
 
Overdose:
 
In case of an overdose, diarrhea, dehydration, cramping in the abdomen, violation of the water-electrolyte balance, hypokalemia are possible.
 
Like other laxatives, this drug with chronic overdose can lead to chronic diarrhea, abdominal pain, hypokalemia, secondary hyperaldosteronism, urolithiasis. In connection with chronic abuse of laxatives, damage to the renal tubules, metabolic alkalosis, and muscle weakness associated with hypokalemia can develop.
 
Treatment
 
There is no specific antidote. Treatment should be symptomatic. Fluid replenishment and electrolyte balance correction may be required, which is especially important for elderly patients and children. It may also be necessary to prescribe antispasmodics.

Interaction

Diuretics or glucocorticosteroids may increase the risk of electrolyte imbalance (hypokalemia) when taking excessively high doses of this drug.

Violation of the electrolysis balance (hypokalemia) can lead to increased sensitivity to the action of cardiac glycosides.

Special instructions

Patients should be advised that they may experience dizziness and / or fainting due to a vasovagal reaction (i.e. during bowel spasm). Therefore, care must be taken when driving vehicles and engaging in other potentially hazardous activities that require increased concentration of attention and speed of psychomotor reactions.

Long-term use can lead to fluid loss, an imbalance in electrolytes, hypokalemia, and dehydration, which may be accompanied by symptoms such as thirst and oliguria. Dehydration can harm the body (for example, with kidney failure, in elderly patients), therefore, with the above symptoms, the drug should be stopped and can be resumed only under the supervision of a doctor.

Patients may experience traces of blood in the feces, this phenomenon is usually mild and disappears on its own.

Patients taking this drug experienced dizziness and fainting. Analysis showed that these cases are associated with fainting during bowel movements (or fainting caused by tension during bowel movements), or with a vasovagal reaction to abdominal pain, which may be due to constipation, and is not associated with taking the drug.
