Composition

1 g of ointment contains:

Active ingredients: 

Sodium heparin - 100 ME (0.833 mg with sodium heparin activity 120 IU / mg), 

Benzocaine (anestezin) - 40.0 mg, 

Benzyl nicotinate - 0.8 mg;

Excipients: 

Glycerol - 150 mg 

Vaseline - 60 mg 

Stearic acid (stearic acid 50) - 50 mg, 

Peach oil - 50 mg, 

Emulsifier No. 1 - 50 mg, 

Methyl parahydroxybenzoate - 1.5 mg, 

Propyl parahydroxybenzoate - 0.5 mg, 

Water - up to 1 g.

 

pharmachologic effect

Combined preparation for external use, the effect of which is due to the properties of its constituent components. 

Heparin is a direct-acting anticoagulant, belongs to the group of medium molecular weight heparins. 

When applied externally, it has a local anticoagulant, antiexudative, moderate anti-inflammatory effect. 

Blocks the formation of thrombin. Heparin reduces the inflammatory process and has an anticoagulant effect, indirectly improves microcirculation, which contributes to the process of resorption of hematomas and blood clots and reduces tissue edema.

Benzyl nicotinate is a peripheral vasodilating agent that improves microcirculation. Used as an adjuvant to improve heparin penetration.

Benzocaine has a local anesthetic effect. 

Reduces the permeability of the cell membrane for sodium ions, displaces calcium ions from receptors located on the inner surface of the membrane, and blocks the conduction of nerve impulses. 

Prevents the appearance of pain impulses in the endings of the sensory nerves and their conduction along the nerve fibers.

Indications

  • prevention and treatment of superficial vein thrombophlebitis;
  • post-injection and post-infusion phlebitis;
  • external hemorrhoids;
  • inflammation of the postpartum hemorrhoids;
  • trophic ulcers of the leg;
  • elephantiasis;
  • superficial periphlebitis;
  • lymphangitis;
  • superficial mastitis;
  • localized infiltrates and edema;
  • injuries and bruises without violating the integrity of the skin (including muscle tissue, tendons, joints);
  • subcutaneous hematoma.

Contraindications


Hypersensitivity to drug components; violation of the integrity of the skin of the affected area (open infected wounds, ulcerative necrotic processes in particular); children's age up to 1 year.

With care

Disorders of the blood coagulation system, thrombocytopenia.

Side effects


With prolonged use of the drug, local reactions are possible in the form of skin hyperemia, decreased sensitivity at the site of application, allergic reactions (skin rash, itching).

 

How to take, course of administration and dosage

Outwardly. 

The ointment is applied in a thin layer to the affected area (at the rate of 0.5-1 g (2-4 cm of the ointment) for an area with a diameter of 3-5 cm) and gently rubbed into the skin. 

The ointment is used 2-3 times a day every day until the inflammation disappears, on average from 3 to 7 days. The possibility of a longer course of treatment is determined by the doctor.

In case of thrombosis of external hemorrhoids, rectal tampons are used, the ointment is applied to a coarse calico or linen pad, which is applied directly to the thrombosed nodes and fixed. 

A swab soaked in ointment is inserted into the anus. The ointment should be applied daily until symptoms disappear, on average 3-14 days.

Ointment from white to white with a yellowish tinge.

Functional features


Heparin is slightly absorbed from the skin surface. The connection with plasma proteins is up to 95%, the volume of distribution is 0.06 l / kg. Does not pass through the placenta and into breast milk.

It is intensively captured by endothelial cells and cells of the mononuclear-macrophage system, concentrated in the liver and spleen.

It is metabolized in the liver with the participation of N-desulfamidase and platelet heparinase, which is involved in heparin metabolism at later stages.

Desulfated molecules under the influence of renal endoglycosidase are converted into low molecular weight fragments.

Half-life (T1 / 2) - 1-6 hours (on average - 1.5 hours); increases with obesity, hepatic and / or renal failure; decreases with pulmonary embolism, infections, malignant tumors.
It is excreted by the kidneys, mainly in the form of inactive metabolites. Not excreted by hemodialysis.

Benzyl nicotinate penetrates into all layers of the skin. It practically does not enter the systemic circulation.

When applied externally, benzocaine is practically not absorbed.

Ointment for external use.

25 g in aluminum tubes or in polyethylene laminate tubes.

Each tube, along with instructions for medical use of the drug, is placed in a cardboard box.

Special instructions


With prolonged use of Heparin ointment in large areas and the simultaneous administration of oral drugs that affect the blood coagulation system and / or hematopoiesis, the clotting time and prothrombin time should be monitored.

If a long course of treatment is required and with extensive lesions in pregnant women and in women over the age of 65, similar control is recommended.

Should not be applied to open wounds in the presence of purulent processes.

The use of the ointment is not recommended for deep venous thrombosis.
