pharmachologic effect

Pharmacotherapeutic group:

treatment for chronic alcoholism
ATX Code: V03AA

Pharmacological properties

Pharmacodynamics

The action of cyanamide is based on the blockade of acetaldehyde dehydrogenase, which is involved in the metabolism of ethyl alcohol. This leads to an increase in the concentration of ethyl alcohol metabolite - acetaldehyde, which causes negative sensations (flushing of the face, nausea, tachycardia, shortness of breath, etc.), which make drinking alcohol extremely unpleasant after taking the drug. This leads to a conditioned reflex aversion to the taste and smell of alcohol.

The sensitizing effect of cyanamide on alcohol appears earlier (after about 45-60 minutes) and lasts shorter (about 12 hours) than the effect of disulfiram.

Indications

It is used to treat patients with chronic alcoholism and relapse prevention.

Contraindications

Severe heart disease, respiratory and kidney diseases with decreased functions, severe liver disease, pregnancy, breastfeeding, increased individual sensitivity to the drug.

Side effects

As a rule, the drug is well tolerated if alcohol is not consumed.

Fatigue, drowsiness, skin rashes, tinnitus, transient leukocytosis are sometimes possible.

Interaction

MIZO is not prescribed together with metronidazole, isoniazid, phenotoin and other dehydrogenase aldehyde inhibitors, which can enhance the reaction with alcohol.

After taking disulfiram, before the appointment of MIZO, there should be a break of at least 10 days.

MIDZONE is not compatible with aldehyde group preparations (paraldehyde and chloral products).

How to take, course of administration and dosage

Treatment is prescribed after a thorough examination of the patient and a warning about the consequences and complications.

The drug is administered orally by 12-25 drops (36-75 mg of cyanamide) 2 times a day with an interval of 12 hours according to an individual scheme.

Overdose

Symptoms of an overdose are not described, an increase in side effects is possible. In case of taking too large doses of the drug, gastric lavage and the appointment of symptomatic treatment aimed at maintaining the functions of the liver, respiration and cardiovascular system are recommended, and hematocrit and, if necessary, blood transfusion are also required.

Special instructions

Alcohol reaction

While taking the drug with alcohol, the following symptoms appear: severe hyperemia of the skin, "pulsation" in the head and neck, nausea, tachycardia, difficulty breathing, weakness, blurred vision, profuse sweating, chest pain. In the most severe cases - vomiting, lowering blood pressure, respiratory depression, collaptoid state.

The nature and severity of the symptoms depends on the amount of alcohol and drug taken. In severe cases, it is necessary to conduct symptomatic therapy aimed at maintaining the functions of the respiratory and cardiovascular system, intravenous infusion of antihistamines.

MIZO should be used only under the supervision of a doctor and with the knowledge of the patient. There may be a reaction to alcohol contained in some drugs and foods.

It is possible to start treatment of MIDZ only after 12 hours from the moment of the last alcohol intake. Cyanamide should be used with caution in cases where a reaction to a concomitant use with alcohol can entail a risk to the patient’s health: with hyperthyroidism, diabetes mellitus, epilepsy, cardiovascular diseases, and kidney diseases.

With long-term use, it is recommended to monitor thyroid function at least once every six months.
