Structure

100 g of ointment contains:

Active substance:

prednisolone - 0.5 g;

Excipients :

 glycerol (distilled glycerin),

medical petroleum jelly,

stearic acid,

emulsifier No. 1,

methyl parahydroxybenzoate (nipagin),

propyl parahydroxybenzoate (nipazole),

purified water.

pharmachologic effect

Prednisolone ointment has a pronounced local anti-inflammatory, anti-allergic, anti-exudative effect. The anti-inflammatory effect of prednisone in the skin and other tissues is achieved mainly with the participation of the cytosolic receptors of glucocorticosteroids.

The hormone-receptor complex, penetrating into the nucleus of the target cell of the skin (keratinocytes, fibroblasts, lymphocytes), enhances the expression of genes encoding the synthesis of lipocortins, which inhibit phospholipase A2 and reduce the synthesis of arachidonic acid metabolism products - cyclic endoperoxides, prostaglandins and thromboxane.

The antiproliferative effect of prednisolone is associated with inhibition of the synthesis of nucleic acids (primarily DNA) in the cells of the basal layer of the epidermis and fibroblasts of the dermis. The antiallergic effect of the drug is due to a decrease in the number of basophils, direct inhibition of the synthesis and secretion of biologically active substances.

Indications

Urticaria, atopic dermatitis, diffuse neurodermatitis, lichen simplex chronicus (limited neurodermatitis), eczema, seborrheic dermatitis, discoid lupus erythematosus, simple and allergic dermatitis, toxidermia, erythroderma, psoriasis, alopecia; epicondylitis, tendosynovitis, bursitis, periarthritis of the shoulder scapula, keloid scars, sciatica.

Contraindications

Bacterial, viral, fungal skin diseases, skin manifestations of syphilis, skin tuberculosis, skin tumors, acne vulgaris, rosacea (possible exacerbation of the disease), pregnancy.

Side effects

Steroid acne, purpura, telangiectasia, burning and itching of the skin, irritation and dryness of the skin;

with prolonged use and / or when applied to large surfaces, the manifestation of systemic side effects is possible, the development of hypercortisolism (in these cases, the ointment is canceled);

with prolonged use of the ointment, the development of secondary infectious skin lesions, atrophic changes, hypertrichosis is also possible.

Interaction

Prednisolone ointment enhances the anticoagulant effect of anticoagulants.

Increases the likelihood of bleeding while taking salicylates. Increases the possibility of electrolyte metabolism disorders while taking diuretics. Increases the risk of intoxication with cardiac glycosides.

Reduces the hypoglycemic effect of antidiabetic drugs and reduces the antibacterial activity of rifampicin. It is not recommended to combine with antacids.

How to take, course of administration and dosage

The ointment is applied in a thin layer to the affected areas of the skin 1-3 times a day. The duration of the course of treatment depends on the nature of the disease and the effectiveness of therapy, usually 6-14 days. If necessary, treatment can be continued. 
Occlusive dressings can be used on limited lesions to enhance the effect.

When using the ointment in children from 1 year of age and older, it is necessary to limit the total duration of treatment and exclude measures leading to increased resorption and absorption of the steroid (warming, fixing and occlusive dressings).
